Community Support-Adult is a rehabilitative service for persons with mental health or substance abuse needs which focuses on assisting the individual in achieving and maintaining rehabilitative, sobriety, and recovery goals. This service is provided by a team of professional and paraprofessional staff who work closely to ensure that the case management functions and the one-on-one training elements are coordinated and periodically evaluated for effectiveness. The assigned Qualified Professional works with the consumer to develop the Person Centered Plan which identifies strengths and areas of need as well as measurable goals which target those needs. The paraprofessional staff, under the supervision of the Qualified Professional, works on skill acquisition through one on one training with the individual. The daily documentation of interventions and outcomes provides the data needed for the monitoring and tracking of progress and subsequent revision of the plan.
